Is it appropriate to recommend this to patients?  What is a successful approach to using this on post operative areas, if any?  Are there any safety or other concerns with using it?

Possible MassAcademy uses of Dermpedia

I'm interested to explore how we might use the "Community" feature of Dermpedia to create forums for communication and interaction for dermatologists in Massachusetts.

I welcome others' ideas for these forums.

One obvious suggestion is that we could post clinical images and histories to get diagnostic and therapeutic input.  A type of electronic consultation that would have access limited to members of the MAD.   I'll look forward to the first member-uploaded case....

AJCC staging for cutaneous squamous cell carcinoma

Since AJCC staging for cutaneous squamous cell carcinoma was introduced this year, I wonder whether laboratories changed the way they report biopsies of SCC?

This 33 year old male patient presented with this scarring alopecia fir 6 months.It is totally asymptomatic (in lichen planopilaris it is itchy) and clinically there are no scales or redness.The typical foot print on the snow is seen.

TWO 3 mm punch biopsies were done from a centre of the scar and from an adjacent area of the normal hair follicles(supposed to be active).There was no difference histopathologically and the biopsy of the perinormal skin did not show any inflammation.Inflammation is usually absent in both ols and newly formed scard in pseudopelade of Brocq.

dermatopathology fellowship

I am a residnet persuing a dermatopathology fellowship. My journey is going to be long and arduous. Dermpedia is providing me with so much useful information. I can not thank the contributors enough for the wealth of knowledge they are providing. The site is contstantly being updated and I love the new changes.

small lentiginous melanocytic proliferations in older patients (60 +)

I'm reluctant do diagnose these as nevi, but they are small, well-demarcated and clearly not melanoma. In the absence of cytologic atypia and other features of LM, how do others diagnose such lesions?

A biopsy from a patient with reflex sympathetic dystrophy

This is not something that I have seen before. The biopsy was sent in by a rheumatologist from the left leg of 20 year old female. Interestingly, the rheumatologist said nothing else on the requisition sheet. So I got the chart. It turns out the patient has reflex sympathetic dystrophy (complex regional pain syndrome, type I) and has been seen and treated for years in the Pain Clinic here. And it turns out the patient has leg ulcers in the affected limb and the biopsy was taken to "rule out vasculitis".

My DermPath peripheral brain

When I was an intern in Medicine we all carried little notebooks with what we thought was important information to have ready at hand.  By the end of the year our pockets bulged with a proliferation of these little gems. So I've been sort of doing the same type of thing for dermatopathology since about 1990.  When I was in academia, I had some notion that maybe I could work it into something useful to more practioners that just me.
