A suprising finding on a biopsy to rule out basal cell carcinoma.

A 70 year old white female with a history of multiple non-melanoma skin cancers presented with an erythematous 6 x 4 papule. Clinical considerations included basal cell carcinoma, persistent insect bite reaction and a lymphoid infiltrate.

Diagnosis: 
Solitary mastocytoma
Comment: 

The sections show suerficial dermal proliferation of mast cells (confirmed by Giemsa stain and c-kit expression). In the context of a single lesion, the features are best interpreted as a solitary mastocyytoma.
