Pretibial myxedema

Clinical Features:

  • Seen in patients with treated hyperthyroidism
  • Elevated knobby plaques or nodules with a waxy surface on the shins and occasionally dorsal feet 

Histologic Features:

  • Expansion of the dermis, most prominently the deeper dermis, by abundant mucin separating the collagen bundles (strongly Alcian blue positive)
  • Fibroblasts appear to float in edematous areas.
  • Lymphatic dilatation may be present.
  • Fibrosis of the deep dermis and subcutis in older lesions
