To create a new book page click on the 'books' link in the main menu.  On the left is a link to "create a book or book page'.  Click on this and you will be presented with a form.  The key step here is the select box under the label "Parent".  This is where you select the location of this book page, so choose the place in the heirarchy where you want the page to appear. 

If you select <top-level> then you create a new book.  If you make a mistake you can always come back and edit this.

Fill in the details for the book page and hit submit.
